Triage, Assessment and Screening Clinic (TASC)

The Triage, Assessment and Screening Clinic (TASC) is the entry point for obtaining outpatient specialty mental health services for Medi-Cal eligible adults age 18 and over, including older adults age 60 and older.

TASC determines if a member meets medical necessity that qualify for receiving specialty mental health services so that they can be referred to the appropriate clinic or community resource based on their needs. Services may include medication support, individual therapy, group rehabilitation, case management, and linkage to community supports. TASC uses a no wrong door approach so if a member is not eligible for BHS services, warm hand-offs to community supports are coordinated.

TASC supports the following BHS programs: Community Adult Treatment Services (CATS), Black Awareness Community Outreach Program (BACOP), Transcultural Clinic (TCC), Southeast Asian Recovery Services (SEARS), La Familia, Older Adult Services (OAS), and Transition Age Youth (TAY).

Transition Navigation Team (TNT)

The Transition Navigation Team (TNT) provides support for those transitioning from the juvenile to adult system of behavioral health care, for those transitioning into conservatorship through the Office of the Public Guardian and for those at risk of homelessness. Eligible program participants are San Joaquin County residents with a severe mental illness who may or may not be Medi-Cal eligible.

Specific services include assistance with placement into licensed board and care facilities and with TCF vouchers, applying for Social Security Insurance and Medi-Cal benefits, and linkage to other community resources. Additional services include case management, one-to-one counseling as needed, advocacy, and peer support.

Case managers refer their members to the TNT program. Referrals are reviewed weekly and accepted on an ongoing basis.

The objective of the TNT program is to promote individual stabilization in the least restrictive environment

Transition age youth (TAY)

The BHS transition age youth (TAY) team provides services to young adults age 18 to 25 who have a severe mental illness, are willing to receive services, are Medi-Cal eligible, and meet medical necessity. The program assists in integrating and navigating TAY into adult mental health services as well as providing referrals to other resources. Program elements include outreach, engagement, linkage to other services, and member-centered rehabilitation interventions.

The objective is to assist young adults who have a mental illness in becoming as independent as possible while reaching developmental milestones like independent housing, education, employment, and acquiring healthy coping skills and interpersonal relationships. The goal is to provide resources, strategies, and consistency to assist young adults in being able to successfully transition into the adult world and to reduce the stigma of having a mental health diagnosis.

The TAY Full Service Partnership (FSP) is connected to a continuum of care that includes higher and lower levels of service that meet the needs of the individual, as well as the above listed program elements

FSPs offer intensive case management and outpatient services to improve member stability, self-sufficiency, safety and recovery, and maintain members in outpatient treatment.

Members may be decompensating, gravely disabled, or unable to care for themselves and meet one of the following criteria: homeless or at imminent risk of homelessness, have a history of criminal justice involvement, have frequent emergency room or crisis contacts, or be at risk of institutionalization. They may also have a history of reluctance to participate in traditional mental health treatment, have had multiple living situations, or a history of involvement in the criminal justice system.

Services and Support

The Services and Support program provides short-term case management to those who are only receiving medication support and are not engaged in behavioral health services. Medi-Cal eligible San Joaquin County residents who are 18 years and older qualify to participate in this program.

Services and Supports assess referrals from those recently discharged from the Psychiatric Health Facility (post-PHF clinic), from psychiatric hospitals, or those who live independently and would benefit from linkage to BHS services and other community resources. Services range from transportation to long-term case management.

Program goals are to connect members to the appropriate level of behavioral health services.
